Please Note: Effective September 16, 2010, the Legacy Portland Hospitals (LPH) Medical Staff was divided into the Legacy Emanuel Medical Center Medical Staff which includes Randall Children's Hospital and the Legacy Good Samaritan Medical Center Medical Staff. The dates reflected on the verification include membership and clinical privileges with the LPH Medical Staff.  A blank appointment letter will be produced if Provider at Facility had Disaster Privileges as these are not reported out.

For requests that can't be fulfilled at this site, fax the request to 503-525-7652 and indicate that the "requested information is not available online."
